All students must submit the appropriate documentation before their first day of school!

Applicants will have access to the document submission portal once they receive an enrollment offer. Enrollment offers will begin to go out after the Lottery on May 4th, 2026.

Applicants with incomplete records run the risk of losing their seat.

  • Immunization Record (TK - 8th)

  • Home Language Survey (TK- 8th)

  • Photocopy of Parent/Guardian Identification (TK - 8th)

    • Ex. State ID/Driver’s License, Passport, or Military ID

  • Proof of California Residency (TK - 8th)

    • Ex. Utility bill, pay stub, rental agreement/property lease or mortgage statement,

  • Oral Health Assessment (TK or NEW K / 1st)

  • Report of Health Examination (TK or NEW K / 1st)

    • See Required Documents Packet for form.

  • TB Skin Test (TK or NEW K / 1st)

  • Additional Documents (TK-8th)

    • Ex. Medical Authorization Forms, IEP/SST/504 Plan, Court Orders

NOTE: All information regarding your student’s enrollment will be emailed to the email address provided in their application. All documents must be submitted together, incomplete packets will not be accepted.
